VieMed's mission is to provide respiratory and chronically ill patients access to high-quality home-based care, allowing them to live longer and healthier lives. This is made possible by a highly effective home treatment model. Respiratory care therapists deliver therapy, education, and counseling to patients in their homes utilizing the most effective technology available. At the same time, patient care coordinators work with hospitals, ACOs, SNFs, and other referral sources to simplify the transitioning of patients from hospital to home. This achieves better management of length of stay and readmission rates to hospitals, which not only reduces costs for patients but also allows them to stay in their home longer and feel more comfortable.
